SUMMARY OF POSITION

The Peer Support duties are performed under the supervision of a licensed team leader.  Peer assists the consumers with the development of interpersonal skills and community coping skills through education, training and modeling.  The staff may be required to provide assistance in crisis situations.

ESSENTIAL FUNCTIONS:

  1. Under close to general supervision and according to established procedures, performs confidential record keeping functions for the peer office.
·         Utilizes the multi-disciplinary assessment to participate in the development of a comprehensive individualized service/recovery plan with a focus on recovery.
·         Link and/or serve as advocate with other agencies to gather needed resources for consumers
·         Provides assessment, monitoring and assistance to the consumer with the self management of psychiatric symptoms
·         Provides assistance and support in crisis situations
·         Completes necessary documentation (treatment plans, authorizations, etc.) in time frame specified by policy.
·         Assist the consumer in reaching the skill building objective in their individualized service/recovery plan.
·         Actively participates in multidisciplinary treatment/recovery clinical team meetings each month
·         Develops individualized discharge plans through utilization of available resources as deemed appropriate by treatment /recovery team.
·         Communicates suggestions and/or complaints through appropriate channels
·         Displays flexibility, cooperation and a positive attitude
·         Participates in Programs Performance Improvement Plan
·         Participates in continuing education activities
·         Completes assigned tasks with minimal supervision
·         Requests direction and supervision when appropriate
·         Adheres to safety and infection control guidelines
·         Reports to work on time and as scheduled
·         Follows established policies and procedures for calling in for unscheduled absences
·         Uses initiative and judgment to assure that circumstances requiring attention are referred as required.
·         Adheres to dress code

·         Must adhere to and respect all policies regarding all consumer rights, anonymity and confidentiality of all records.  This covers any written or verbal communication regarding a consumer’s identity, address and situation.

Minimum Qualifications:
§  High School diploma or GED
§  Must have been enrolled in peer program and completed program
§  Computer skills
Knowledge:
§  Ability to communicate well by ( oral, written and active listening)
§  Ability to maintain confidentiality
§  Ability to provide excellent customer service


Skills/Competencies:

Prioritization/planning/organizational skills, resilience. Normal mobility, manual dexterity, normal visual and auditory abilities required. Must be able to speak, read, and write the English language.

Other Requirements:

§  Maintains current and valid State of Georgia State Commercial Driver’s license
§  Must have annual MVR (motor vehicle report) with 7 years retroactive driving record that meets ActionMed’s Auto Liability Carrier guidelines.
§  Has a vehicle available for work
§  Meets AMP/HFR/State criteria established for residential care employees in the State of Georgia (including but not limited to Criminal background check, MVR guidelines, Finger-print, Employment reference check and Drug Free Workplace Policies.)
