2 Full-time Positions Available at Willowbrooke at Floyd for Certified Peer Specialists



We are seeking 2 Certified Peer Specialists to assist our client population in Northwest Georgia. Our mental health professionals at Willowbrooke at Floyd help patients build the self-esteem and self-reliance that they need to live happily and fully when the problems of everyday life seem overwhelming. After patients have completed a partial hospitalization program, they may still need intensive but less extensive counseling and our outpatient programming provides this. Willowbrooke at Floyd has been approved by the Georgia Department of Behavioral Health and Developmental Disabilities as a Core provider and is authorized to provide the following services:
  • Individual therapy
  • Group therapy
  • Family therapy
  • Substance abuse services
  • Diagnostic assessments and recommendations
  • Psychiatric evaluations and medication services
  • Treatment planning
  • Nursing services
  • Crisis intervention
  • Skills training
  • Community support/case management services

Community Support Specialist

Summary:

The Community Support Specialist will be responsible for the coordination of services to children and youth and their families in the community. They must have the ability to provide services in a variety of settings: schools, DJJ, detention centers, consumer’s home, etc. Community Support Specialist services consist of rehabilitative, environmental support and resources coordination considered essential to assist a child and family in gaining access to necessary services and in creating environments that promote resiliency and support the emotional and functional growth and development of the child.


Job Responsibilities:
*         Facilitates family and individual sessions with consumers and consumer’s family in the home, school, or community setting.
*         Conducts initial evaluations and assessments of consumers and their families.
*         Will perform home-visits with consumers and their families to conduct family or individual sessions.
*         Assistance to the child youth and family responsible caregivers in the development and coordination of the Individual Resiliency Plan (IRP).
*         Planning in a proactive manner to assist the child youth and family in managing or preventing crisis.
*         Assistance with personal development and school performance.
*         Assistance in enhancing social and coping skills that ameliorate life stresses resulting from the consumer’s emotional disturbance.
*         Service and resource coordination to assist the child youth and family in gaining access to necessary medical, social and other service supports.
*         Assistance with youth family in understanding illness and the self-management of symptoms
*         Monitoring and follow-up to determine if the services assessed have adequately met the consumer’s needs.
*         Identification of risk indicators related to substance related disorder relapse and relapse prevention plans.
*         May perform therapeutic holds on patients as needed.
*         Responsible for the ongoing identification of consumer needs for discharge and seeking out community resources for discharge plans for consumers.

Required Knowledge and Skills:

Education: Bachelor's Degree

Experience:  Three years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.

Licenses and Certifications: Certified Peer Specialist

Qualifications:
*         Proven ability to effectively work with others at all levels within and outside Willowbrooke.
*         Leadership, sound judgment and ability to organize.
*         Ability to interact with persons in a therapeutic community.
*         Minimal experience in provision of services.



Recovery Facilitator

Summary:

The Recovery Facilitator’s role is to provide skills training and support as part of Willowbrooke and to promote self-determination, personal responsibility and empowerment inherent in self-directed recovery. Under immediate to general supervision, the Recovery Facilitator provides recovery support services and serves as a consumer advocate. Provides consumer information and peer support for consumers in a variety of settings. The employee performs a wide range of tasks to support consumers in living their own lives and directing their own recovery and wellness process. The facilitator will model competency in recovery and wellness.

Job Responsibilities:
*         Attends treatment team and discharge planning meetings at all levels of care within the Willowbrooke continuum.
*         Provides case management for patients whose discharge plans include transitioning to a different level of care within the Willowbrooke continuum.
*         Monitors and follows-up to determine if the recommended services have adequately met the consumer’s needs.
*         Works to ensure all Willowbrooke internal referral a successfully admitted in to the recommended level of care.
*         Communicates barriers following discharge recommendations to the appropriate clinical staff.
*         Establishes and maintains positive relationships with patients, physicians, allied health professionals, and all referral sources.
*         Works cooperatively with Program Manager in program development and facilitates skill and support groups for identified patients at various levels of care.
*         Responsible for the coordination of patient care through the following duties:
o    Assessment: The facilitator will collect required information about a person's situation and functioning to develop a case management plan to address those needs. Information will be gathered from the treatment team and other relevant sources.
o    Planning: The facilitator determines short term specific objectives, goals, and actions designed to meet the client's needs. The plan should be designed with efficiency, cost effectiveness and outcomes in mind. The plan should support individual empowerment and self-reliance. Contingency plans should be present in anticipation of treatment and service changes.
o    Implementation: The facilitator will execute specific case management activities or interventions to achieve the goals set forth in the case management plan.
o    Service Excellence: The facilitator works with others within and without the organization to ensure the coordination and continuity of care, and to maintain open communication and collaboration with other health care givers, the client patient and his support group.
o    Coordination: The facilitator organizes, secures, and integrates the resources needed to reach the goals set forth in the case management plan. The case manager will focus on the health care needs and promote the timely provision of care and efficient resource utilization.
o    Monitoring: The facilitator continues to process and gather sufficient information from relevant sources to determine the case management plan's effectiveness.
o    Evaluation: The facilitator continues to evaluate the case management plan's effectiveness to reach the desired outcome.

Required Knowledge and Skills:

Education: Bachelor's Degree

Experience:  No prior work experience.

Licenses and Certifications: Certified Peer Specialist

Qualifications:
