· Collaborate with the ACT Team to develop Psychiatric/Social Functioning History Timelines, Treatment Plans, Weekly Consumer Schedules, and Daily Team Schedules, and participate in treatment planning meetings and daily team meetings in order to monitor client progress and coordination of care with others.
· Share personal, practical experience to benefit ACT Team staff and clients.
· Provide expertise about symptom management and the recovery process.
· Provide peer counseling to ACT clients and their families/supporters.

Experience/Requirements: High School diploma or equivalent. Must be a mental health consumer who is a current or former client of the ACT Program or a consumer of any County mental health system program and can provide peer counseling and mentorship. Attend Peer Employment Training and obtain certification prior to hire. Possess excellent communication and writing skills. Computer skills (MS Word, Excel, County systems, etc.) A valid CPR and First Aid certificate.

Duties & Responsibilities: This position is a member of a team which, when funding is received, will provide Assertive Community Treatment to individuals with severe and persistent behavioral health disorders who have been discharged from multiple or extended stays in public hospitals or who are difficult to engage in treatment. The Assertive Community Treatment (ACT) program utilizes an interdisciplinary approach to provide intensive, integrated and rehabilitative crisis, treatment and community support interventions/services that are available 24 hours per day, 7 days per week to meet the needs of consumers in need of acute care.* The ACT team is based in Lawrenceville, GA, but will serve a three-county service area (Gwinnett, Rockdale and Newton Counties).

The ACT Team Social Services Assistant for Peer Support functions as a peer specialist for adults who are referred for Assertive Community Treatment. Intensive treatment and case management services, provided as part of a comprehensive individual service plan, are designed to serve the acutely-distressed consumer in the community with no delay, preventing hospitalization, incarceration or other placements.

The Social Services Assistant, Peer Support has experience as a recipient of mental health/addictive disease services and is willing to share his/her personal, practical experience, knowledge, and first-hand insight to benefit the client and the team members. Participates fully in team meetings to provide first-hand expertise about the recovery process, symptom management, and the persistence required by consumers to have a satisfying life. Assists consumers with rehabilitation activities, provides peer counseling and consultation, and acts as a liaison with community services. Responsible to maximize consumer choice, self-determination, and decision-making in the planning, delivery, and evaluation of treatment, rehabilitation, and support services. Monitors services and programs and functions as consumer advocate.

*NOTES: The entry salary noted above reflects a 20% pay supplement as a result of the requirement for immediate response to acute needs of the consumer regardless of the hour or day.

The selected candidate will be required as a part of his/her employment to attend the training sessions necessary to become a Certified Peer Specialist unless the candidate already has the certification.

Attend or participate in Senate & House Briefings, as appropriate and other meetings. Be a conduit of communication linking NCMHR state members & Hill offices/staffers.
· Networking in collaboration with the Director of NCMHR and the NCMHR Board members as appropriate to develop relationships that increase our voice and visibility with important federal agencies such as Medicaid and the Substance Abuse and Mental Health Services Administration.
· Monitor and screen legislation and potential sign-on letters, make recommendations as to which legislation or letters the NCMHR should support and/or sign-on to.
· Assist in developing federal legislation and seeing it through all stages to appropriations and implementation.
· Create and maintain a legislative resource guide, and provide technical assistance for our membership to use for activities such as lobbying, implementing legislative days, etc.
· Provide input into the development of the BRSS TACS work plan;
· Assist with the selection of consumer/survivor experts and consultants to participate in Expert Panels to guide and implement the BRSS TACS work plan;
· Assist with the conception of and writing of “policy and practice guideline papers”;
· Select relevant resources for inclusion in the BRSS TACS website and assist with web content development;
· Provide telephonic and online technical assistance as needed;
· Help develop and find experts to participate in training webinars on a variety of topics;
· Provide technical assistance and training to Mental Health Transformation Grantees;

The Zion Hill Community Development Corporation (“ZHCDC”) was incorporated in 2002 as 501(c)3 a non-profit, charitable organization to address the unmet needs of the poor and disadvantaged of our community. The ZHCDC has an established Outreach Center that provides a comprehensive program for women and children experiencing or at risk of homeless as well as low-income families.

The Certified Peer Specialist (CPS) will be a member of the team serving the Permanent Supportive Housing Program. The program is designed to provide housing and supportive services to chronically homeless women age 55 and over.

The CPS will follow best practices to provide services that foster supportive peer relationships with and among the persons enrolled in the Supportive Housing Program. The CPS by definition is a person in recovery who will utilize her or his personal recovery experience to instill a sense of hope and optimism for persons in recovery. They will demonstrate a personal belief in recovery and a sincere interest in the welfare of persons in recovery. Applicants must have excellent interpersonal skills and the ability to work under pressure.

Responsibilities:

The CPS will be responsible for (1) assisting their peers in skill building, goal setting, problem solving, conducing Recovery Dialogues, setting up and sustaining mutual self-help groups, and in helping their peers build their own self-directed recovery tools; (2) understanding the structure of the Georgia mental health system, client rights, cultural competency, confidentiality and the documentation requirements for progress notes; (3) promoting hope, personal responsibility, empowerment, education, and self-determination in the residents in the Supportive Housing Program; (4) providing peer support services and serving as liaison to the residents in accordance to the approved resident’s individualized treatment plan; (5) linking residents to community supports and resources; (6) attending agency and state mandated meetings; (7) documenting and submitting all face-to-face/phone contact with residents and/or collateral contacts within 24 hours of contact; (8) attending meetings, training functions and other official ZHCDC functions as required; (9) meeting with residents and staff to plan and design schedules; (10) following up with the ZHCDC residents between weekly sessions to ensure retention and engagement in the Supportive Housing Program; (11) transporting residents as needed; (12) complying with ZHCDC policies and procedures; (13) serving in a variety of other services designed to assist the residents in reaching the goals they wish to accomplish in their personal recovery journeys and other duties as assigned.

• Must have Peer Support Certification. No work history or other education can be considered in lieu of this requirement.
• A self-identified current or former user of mental health or co-occurring services who can relate to others who are now using these services.
