The Assertive Community Treatment (ACT) program utilizes an interdisciplinary approach to provide intensive, integrated and rehabilitative crisis, treatment and community support interventions/services that are available 24 hours per day, 7 days per week to meet the needs of consumers in need of acute care.* The ACT team is based in Lawrenceville, GA, but will serve a three-county service area (Gwinnett, Rockdale and Newton Counties).&lt;br /&gt;&lt;br /&gt;The ACT Team Social Services Assistant for Peer Support functions as a peer specialist for adults who are referred for Assertive Community Treatment. Intensive treatment and case management services, provided as part of a comprehensive individual service plan, are designed to serve the acutely-distressed consumer in the community with no delay, preventing hospitalization, incarceration or other placements.&lt;br /&gt;&lt;br /&gt;The Social Services Assistant, Peer Support has experience as a recipient of mental health/addictive disease services and is willing to share his/her personal, practical experience, knowledge, and first-hand insight to benefit the client and the team members. Participates fully in team meetings to provide first-hand expertise about the recovery process, symptom management, and the persistence required by consumers to have a satisfying life. Assists consumers with rehabilitation activities, provides peer counseling and consultation, and acts as a liaison with community services. Responsible to maximize consumer choice, self-determination, and decision-making in the planning, delivery, and evaluation of treatment, rehabilitation, and support services. Monitors services and programs and functions as consumer advocate.&lt;br /&gt;&lt;br /&gt;*NOTES: The entry salary noted above reflects a 20% pay supplement as a result of the requirement for immediate response to acute needs of the consumer regardless of the hour or day.&lt;br /&gt;&lt;br /&gt;The selected candidate will be required as a part of his/her employment to attend the training sessions necessary to become a Certified Peer Specialist unless the candidate already has the certification.&lt;br /&gt;&lt;br /&gt;Minimum Training &amp;amp; Experience: Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities (High school diploma or GED and eligibility for certification as Peer Support Specialist (consumer of behavioral health services and experience in consumer advocacy, advisory or governance.) The peer specialist, as a former recipient of mental health services for severe and persistent mental illness, should have self-knowledge to manage their mental illness and be well along in their recovery).&lt;br /&gt;&lt;br /&gt;Preferred Qualifications: Bachelor's degree in social work, psychology, counseling or a closely related field.
